Online application for Form 6 in Delhi
1.Go to Enroll Online http://ceodelhi.gov.in/Content/home.aspx on the home page

2.For new users registration is required to get the log in and you need to have username and password after signup
3.Passport size color photo needs to attached in the Photo section in Form
4.Upload the following documents in the space mentioned in the Form.
Uploading is optional.
In case you are not able to upload the documents below then the Booth Level Officer (BLO) who will come to your residence to verify the information and will collect both the documents from you
The documents are -
1. Proof for your residence ( driving license or passport or bank pass book or any postal letter received on your address in your name).
If you are a student residing in a Hostel of your University / Institute or elsewhere (e.g. PG) etc., then please get a Declaration in the proforma at Annexure One (click here also)to Form 6. A scanned copy can also be uploaded.
2) Age proof doc - birth certificate or school leaving certificate
However this is only required if you are between 18 and 21 years of age.
Down load Form 6 from the website and send it by post.

2. You will get a number of choices, click onForm 6: Application for Inclusion as a New Voter for the First Time.
3.Fill the downloaded form 6 and attach one passport size photo
4. - Sign the Form and attach the following :
i) Proof of your residence - driving license or passport or bank pass book or any postal letter received on your address.
b) Age Proof - required only if you are between 18 and 21 years of age ( such as birth certificate, school leaving certificate etc.)
5. Send the Form and documents by post to the VREC (Voters’ Registration and EPIC Centre) of your Assembly Constituency.
